Risk Description
LLM faithfulness to input significantly degrades for information in middle positions of long contexts (the "lost in the middle" phenomenon). Models tend to over-rely on information at the beginning and end of context while ignoring or distorting key content in the middle. In document summarization, contract review, or long-form analysis, this may cause critical clauses and data to be missed or misrepresented.
